Abstract:
Background: Balanitis Xerotica Obliterans (BXO) with meatal stenosis is a very morbid
condition. Treatment of meatal stenosis due to balanitis xerotica obliterans is very challenging
because of high recurrence rate. Various type of surgical treatment options are practice for
BXO with meatal stenosis. Proper treatment is necessary to give a free relief from the
sufferings.
Objectives:
The purpose of this study was to evaluate the outcome of combined dorsal and
ventral buccal mucosal graft meatoplasty with dorsal meatoplasty in the treatment of Balanitis
Xerotica Obliterans with meatal stenosis.
Methods:
This prospective quasi experimental study was carried out in the department of
urology, Dhaka Medical College and Hospital, Dhaka, Bangladesh from July 2013 to june 2015
for a period of twenty four (24) months. All patients presented with Balanitis Xerotica
Obliterans with meatal stenosis. Patients were divided into two groups designated as group
1 and group 2 for combined dorsal and ventral buccal mucosal graft meatolplasty and dorsal
graft meatoplasty. Post-operative follow up was carried out six months.
Results:
Among 60 patients 30 were in group1 and the rest 30 patients were in group 2. In
group 1, the mean ± SD of operative time was 101.17 ± 16.27 minutes. Six months after in
group 1, mean ± SD of q max was was19.37 ± 4.79ml/sec and in group 2, mean ± SD of q
was 16.16 ± 3.69 ml/sec (p<0.05). In group 1, mean ± SD of voiding time was 29.33 ± 8.16sec.
In Group 2, mean ± SD of voiding time was 32.73 ± 13.83ml/sec (p>0.05) in group 1, success
was in 25 (83.3%) cases where as in group 2, it was in 19 (63.3%) cases.
Conclusion:
However, a prolonged longitudinal follow up is essential for combined dorsal
and ventral buccal mucosal graft meatoplasty is better than dorsal graft meatoplasty.
